There are different types. The RCA one has financial penalties for failing to show to BD. Also it legally bind them to BDs determinations.

I have heard people involved who say it has helped a lot. I suspect it depends on different communities.

In really short, w/o LH:
The RY's son "X" marries a brisker girl with the understanding that he's going to be stayng in EY forever bec briskers wont go to the US based on something the Rav said.
Which is not really a problem, bec he has another brother who's being groomed to be the future RY of The Yeshiva.
The brother dies young from lung cancer, and the father (RY) also dies relatively young.
"X" is told by the gedolim in bnei brak+ that he must go back to the US to take the reigns of the yeshiva,
The gedolim of Brisk disagree...
"X" and his wife do not yet have children (though they were married for a few years).
"X" offers his wife a get bec he's going back...
The wife refused the get...
"X" backed by Ponevitch gets a H100R.
Brisk doesn't like it...
